Procedures for Requesting Accommodations

NOTE: Please allow 30 days for setting up accommodations

  1. Students with disabilities who wish to request accommodations or services are responsible for documenting need and should initiate the request with the Disability Services Office.

    ALL REQUESTS MUST BE IN WRITING!

As soon as your written request is received, the Disability Services Office will contact you to discuss your request and gather additional information if deemed necessary. In order to give the Disability Services Office sufficient time to evaluate your request for an accommodation, it is important to send your request in as soon as possible. It may be difficult to provide you with accommodations if the request is not received on a timely basis. It is recommended that your request for semester length services be submitted at least 30 days prior to the first day of class. For services that are required on a sporadic basis during the semester, we recommend that you submit your request at least 72 hours prior to the date the service is required.

  1. Students are responsible for providing current documentation of need in order to accommodate a disabling condition. Documentation must be a comprehensive individual assessment from an appropriate professional (on that person's letterhead) whose license or credentials are appropriate to diagnose the disability.
  2. Submit a signed, notarized confidential release form giving the college permission to release information from the student record for purposes of preparing and coordinating accommodations A fax of the original may be sent to the Disability Services Office; however, the original must be on file prior to the first class day in order to receive services.
  3. Complete and submit the Accommodation Request Form.
