Guidelines for Utilization of Accommodations

Individual Service Agreement

Students with disabilities who enroll in DCCCD Distance Learning Courses and have been approved for services/accommodations must sign an Individual Service Agreement. This agreement shows that the student has accepted the prescribed services and the delivery system for the services, as well as acknowledging his/her responsibilities in utilizing the services.

This form will be mailed to the student prior to the start of the semester in which the student is enrolled and prior to the activation of services.

Student's Responsibility for Utilizing Services

Students with disabilities who enroll in DCCCD Distance Learning Courses will:

  1. Request services in a timely manner;
  2. Provide official documentation of need for services/accommodations to the Disability Services Office;
  3. Complete all required forms and documents for the Disability Services Office;
  4. Make a commitment to participate in scheduled auxiliary services, including attendance at all scheduled activities pertaining to the class the student is enrolled.
